Student-Athlete Medical Forms

Student athletes are required to have a physical before participating in athletics:

  • This form needs to be completed by a Board Certified Physician.
  • Student-athletes that are beginning their initial season of eligibility (first years or transfers) are required to have a physical completed within six (6) months of their sport’s first day of practice.
  • PLEASE BE AWARE: Physical forms from doctor’s offices may or may not contain all of the appropriate information. It is strongly recommended that student-athletes use the Mitchell College athletics form whenever possible.

Click here to download the Mitchell College Athletics Physical Form.

Athletes will also be required to submit a new physical every two years to remain eligible and ensure that they remain healthy enough for competitive athletics.

Beginning with the 2022 Season all NEW student athletes are required to submit sickle cell testing results, athletes will no longer be allowed to waive testing. Athletes can submit results from any previous testing they had done, a new sickle cell test is only needed if they are unable to provide previous results. Test results only have to be submitted ONCE. Returning athletes who have previously waived testing can continue to do so but it is recommended that they submit results.

All other sports medicine forms can be found in the healthy roster app. We utilize this app for sports injury tracking, communication among coaches and athletes and access to other resources. Incoming athletes will be sent an email with the invite to join the app once they are added to the roster. The following forms will be completed through the app:

  • Authorization for release of Medical Information
  • Assumption of Risk
  • Concussion Safety
  • Returning Athlete Medical Update (only required for returners)
  • New Athlete Medical History (Only completed once in first year)

XLNTbrain Baseline Concussion Testing

At Mitchell College we require ALL incoming Freshman and Transfer Student-Athletes to take a Concussion Baseline Exam. For this exam we use the XLNTbrain Baseline.

The XLNTbrain Baseline Test takes approximately 45 minutes and is an important tool for your healthcare provider to use in the determination of a concussion and for your recovery if you do have a concussion. It is important for you to take the test seriously and be in an environment free of distractions as you test. This test is a measure of your baseline prior to a head injury, in order to get an accurate assessment, you need to follow the directions carefully and give the test your full attention. If you don’t follow the instructions correctly or are distracted while taking it, you may get an invalid result and be asked to retake it before you will be allowed to compete.

Please look for an email from the college with further instructions on how to take the XLNTbrain Baseline test. It can be done at home on your personal computer or tablet. If you have any questions, or need assistance in accessing the XLNTbrain website please email Steph or Maddi and they will be able to assist.

Athletic Training Room Policies

The Athletic Training Room is a medical facility and a shared space for ALL Mariner Athletes.
Please help us keep this a SAFE and WELCOMING space for EVERYONE by helping with the following:

  • Let us know when an injury occurs! Healing is a process. The sooner we can help facilitate healing, the quicker we can get you back on the field.
  • Rehab gets you back to 100%. Make every effort to come to rehab appointments ready to work and on time.
  • Respect your fellow athlete’s privacy: always ask permission to take pictures and videos while in the Athletic Training Room.
  • Please leave supplies/equipment/rehab tools in the Athletic Training Room so it’s available to all athletes. If you borrow something, please return it in a timely manner.
  • In-Season athletes and those with acute injuries will have priority for rehab and treatment times.
  • There will be no treatment or rehab appointments after practices or games, only exceptions will be new injuries or recommendations from the staff. This includes no treatment or rehab on weekends unless otherwise directed by a member of the sports medicine team.
  • If for any reason a student athlete is unable to get in for treatment during normal rehab hours they can reach out to the sports medicine team to request an appointment.
  • For sanitary and safety purposes you must shower and wear a clean “outfit” to use the whirlpool. Practice gear from training is NOT a clean outfit. Please bring your own towel when possible.
  • If an injury takes you off the field and/or out of practice you MUST be cleared by the Mitchell College Team Physician or a member of the Sports Med Staff prior to your return to play.
  • This is a small space, let’s do our best to keep it clutter free. Please utilize the cubbies by the front door for bags, shoes, gear, etc.
  • BE RESPECTFUL: While seeking treatment or just visiting the athletic training room please be respectful of your fellow athletes and the sports medicine staff. You will be asked to leave if you interfere in any way with another athlete’s recovery or well-being.
